    Position Summary

    The Care Coordinator (CC) is a non-clinical team member who facilitates patient continuity of care with the healthcare team. Under the clinical direction and oversight of the VP of VBC Operations, Primary Care Physicians and Registered Nurse, the Care Coordinator navigates patients at risk by collaborating with the patient and interdisciplinary healthcare team to serve needs and expedite appropriate, cost-effective care.

    The PCC will support the clinical and operations teams in the care of patients. They will engage patients and take an active role in improving the health of patients we serve. They will partner with the care team in providing the patient with the tools necessary to make healthy lifestyle choices, adopt life-long healthy behaviors, and close gaps in care.

    Duties and Responsibilities

    The duties include, but are not limited to:
    • Assist clinicians on various tasks including follow up care, patient care notification, care gap closure and patient scheduling
    • Responsible for engaging with patients and the care team to ensure patients are scheduled and complete annual wellness visits and follow up appointments as needed, per provider instructions
    • Supports the care plans for the patient and facilitates engagement between the providers and the patients
    • Manages outgoing orders and incoming results and ensures accurate documentation and storage within the electronic medical record
    • Initiates all appointment reminder calls in a professional and courteous manner, projecting appropriate language and tone of voice
    • Demonstrates effective oral, listening, and written communication skills that facilitate positive customer relationships and staff interaction in the delivery of care
    • Prioritizes duties and responsibilities and completes them accurately and in a timely fashion.
    • Demonstrates responsible judgment by updating knowledge, referring problems through appropriate channels, and observing the policies and procedures.
    • Upholds compliance with legal requirements, HIPAA, OSHA, and company policies.
    • Assisting with other office duties including answering phones, faxing, filing, etc.
    • Support individualized patient care plans
    • Address patient risk factors and/or obstacles to care
    • Facilitate engagement between PCP and patient
    • Demonstrated ability to influence others while motivating them to change
    • Exercise sound judgment, seek advice and sign off from RN or LPN, as appropriate
    • Performs miscellaneous job related duties as assigned
    Qualifications or Education, Training and Experience
    • Two (2) years of work experience in one or more of the following settings: Acute Care, Ambulatory, Specialty, Physician Office, Care Management or Home Health Care
    • High school degree required
    • CMA or MA preferred
    • Knowledge of athenaOne preferred
    • Must have valid driver’s license and ability to travel to doctors’ offices throughout NJ

    Knowledge and Skills/Expected Competencies
    • Candidate must be able to follow workflows and work together with RN/LPNs and other multi- disciplinary care team members within a variety of clinical settings
    • Ability to review clinical data to identify care opportunities and healthcare utilization trends
    • Critical thinking skills & able to adapt to fast complex working environment
    • Multitasking and prioritization working in a high-volume environment with minimal supervision
    • Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team
      • Understanding of HIPAA and Confidentiality Obligations
      • Strong knowledge of Payor programs and Quality Metrics
        • Excellent interpersonal and organizational skills
          • Strong autonomous problem-solving ability
          • Excellent time management skills
          • Compassionate and able to handle stress
          • Documentation accuracy in medical record
          • Adaptable to new technologies and software
          • Proficiency in EMR system(s), Outlook and data entry; Aethahealth experience a huge plus

The employee may need to travel to healthcare practices.
