  • Overview

    • The Licensed Agency - ALP Director is responsible for operating the assisted living program, inclusive of agency services provided within the ALP, and executing, through direct performance or coordination, the services and functions as are necessary to meet the needs of the residents.

    This is an exempt position.  Salary $80,000 - $90,000 annually

    Qualifications

    • A bachelor's degree from an accredited college or university in an approved course of study, plus two years of related work experience acceptable to the department, one year of which includes related supervisory experience for services to a dependent adult population; or 
    • An associate degree from an accredited college or university in an approved course of study, plus three years of related work experience acceptable to the department, one year of which includes related supervisory experience for services to a dependent adult population; and
    • Valid licensure as a Registered Professional Nurse in New York State with active registration.

    Responsibilities

    Direct and supervise all care services and other health care activities, including licensed home care services, provided to residents in the Assisted Living Program.

     

    Oversees the provision of case management services to ensure compliance with admission, retention, discharge, and personal care standards. Such services include:

    • initial and periodic evaluation, at least once every 12 months, of the needs of a resident and of the capability of the facility program to meet those needs;
    • orientating a new resident and family to the daily routine;
    • assisting each resident to adjust to life in the facility;
    • assisting each resident to maintain family and community ties and to develop new ones;
    • encouraging resident participation in facility and community activities;
    • establishing linkages with and arranging for services from public and private sources for income, health, mental health and social services;
    • assisting residents in making application for, and maintaining, income entitlements and public benefits;
    • assisting the resident in obtaining and maintaining a primary physician or source of medical care of choice, who is responsible for the overall management of the individual's health and mental health needs;
    • assisting the resident in making arrangements to obtain services, examinations and reports needed to maintain or document the maintenance of the resident's health or mental health, including:
    • health and mental health services;
    • dental services; and
    • medications;
    • providing information and referral;
    • coordinating the work of other case management and service providers within the facility;
    • assisting residents in need of alternative living arrangements to make and execute sound discharge or transfer plans;
    • assisting in the establishment and operation of a system to enable residents to participate in planning for change or improvement in facility operations and programs and to present grievances and recommendations; and
    • investigation of and reporting to the department and, as applicable, to the Justice Center, allegations of occurrences which would constitute a reportable incident.
    • Ensure the timely completion of all required resident assessments and evaluations.
    • Ensure the maintenance of complete, accurate records for each resident.

      Understand and maintain the program in compliance with the regulations of the New York State Department of Health and with applicable statutes and regulations of Federal, State and local jurisdictions. 

      Assist the administrator in personnel management tasks.

      RN on-call responsibilities as assigned. 

      Subject to call-back as necessary. 

      Other duties as assigned.
