Program Director - Home Care directs, supervises, and governs all home care programs including nursing, therapy, home care, and homemaking. Plans, implements and maintains policies and procedures for home care programs to ensure quality patient care and services. Being a Program Director - Home Care may work for a free-standing agency or operate a multi-chain branch. Requires a bachelor's degree. Additionally, Program Director - Home Care typically reports to top management. The Program Director - Home Care manages a departmental sub-function within a broader departmental function. Creates functional strategies and specific objectives for the sub-function and develops budgets/policies/procedures to support the functional infrastructure. Deep knowledge of the managed sub-function and solid knowledge of the overall departmental function. To be a Program Director - Home Care typically requires 5+ years of managerial experience.     DUTIES OF POSITION

    • Provides personal care and related services in the home, under the direction, instruction, and supervision of the Supervising Nurse.
    • Tasks to be performed by an caregiver must be assigned by and performed under the supervision of an RN who will be responsible for the client care/services provided by the caregiver.
    • Under no circumstances may an caregiver be assigned to receive or reduce any intravenous procedures, or any other sterile or invasive procedures.


    POSITION RESPONSIBILITIES

    • Follows the plan of care/service to provide, safe, competent care/service to the client
    • Performs the following tasks as assigned by the RN:
      • Assist with the activities of daily living
      • Medication reminders
      • Taking and recording vital signs
      • Recording and reporting to the supervisor any changes regarding the client’s condition. behavior or appearance
      • Documenting the services delivered in the client’s record
    • Helps the client to maintain good personal hygiene and assists in maintaining a healthful, safe environment
    • Plans and prepares nutritious meals
    • Assists the client with ambulation as approved
    • Encourages the client to become as independent as possible according to the care plan
    • Attempts to promote client's mental alertness through involvement in activities of interest
    • Gives simple emotional and psychological support to the client and other members of the household and establishes a relationship with client and family which transmit trust and confidentiality
    • Completes at least 12 hours of in-service training and education annually
    • Performs routine housekeeping tasks as related to a safe and comfortable environment for the client
    • Prepares a visit report on the day the service/care was provided
    • Confirms on a weekly basis, the scheduling of visits so that other necessary visits by staff members can be coordinated
    • Works with personnel of other community agencies involved in the client's care
    • Confer with the Supervisor about client’s status and care, participate in the planning of service
    • Promptly report any change in client condition to the Supervisor
    • Works within the caregiver’s scope of practice, as directed by a Registered Nurse and report in writing to the Supervising Nurse


    JOB CONDITIONS

    • The ability to drive and the ability to access clients’ homes which may not be routinely wheelchair accessible are required
    • Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the client's condition and to perform client care
    • On occasion, may be required to bend, stoop, reach and move client weight up to 250 pounds; lift and/or carry up to 30 pounds
    • Must be able to communicate clearly, both verbally and in writing


    EQUIPMENT OPERATION

    • Use of BP cuff, thermometer, stethoscope, and hand washing materials


    COMPANY INFORMATION

    • Has access to all client service records which may be discussed with the supervisor.


    QUALIFICATIONS

    • Be able to speak, read and write English
    • Have a valid driver's license, car insurance, and reliable form of transportation
    • Pass a criminal background check
    • Availability to work different shifts, including nights and/or weekends
