Program Director - Home Care directs, supervises, and governs all home care programs including nursing, therapy, home care, and homemaking.     The Advocate Program, Inc. is a private, not-for-profit, 501 c (3) justice agency that offers probation, diversion, community service, DUI and traffic programs, domestic violence programs, substance abuse and mental health evaluations and treatment to court-ordered individuals. It has expanded to work with juveniles and families in the juvenile justice field, families impacted by family conflict and violence, and homeless veterans and their families.

    Responsibilities:

    Multi-Disciplinary/Interagency Care Coordination

    • Coordinate and maintain the continued development and staffing of an interagency, multi-disciplinary care coordination network for veterans, active duty personnel, and their families who will meet monthly to engage in round table discussions on; integrated approaches to client care/best practices, case management, referral services, metrics, and continuous program improvement.
    • Work in cooperation with Miami VAMC, Miami- Dade Homeless Trust, Broward County, and Monroe County Homeless Services to approach key contacts in professional and community groupings throughout Miami Dade, Broward and Monroe Counties to offer educational and staff training opportunities.
    • Create and maintain an effective community resource data base used to identify; metrics, adherence to program goals and objectives, and performance of numerous contractual obligations.
    • Assists in the maintenance of program accreditation through COA.

    PROGRAM MANAGEMENT

    • Hiring, training, and supervision of staff.
    • Supervision of housing case management services
    • Knowledge and experience in the procedures for applying for, securing, and maintaining grant funding.
    • Knowledge of local, state, and federal regulations governing social service programs.
    • Post grant award development.
    • Overall program supervision of metrics, adherence to program goals and objectives, and performance of outreach, case management, and subcontractors.
    • Possess excellent skills in grants compliance, management, and reporting.
    • Assist in launch program expansion: write program policies and procedures, hire and train staff, outreach to the community, and implement operations.
    • Ability to foster professional working relationships with colleagues, clients, community agencies, and the Veterans Administration.
    • Ability to follow program and funding requirements and maintain required documentation.
    • Refer veterans for individual, family and group readjustment counseling and mental health services tailored to meet the needs of; Veterans suffering from Combat-Related PTSD; veterans who need career and employment counseling/guidance; as well as VA Behavioral Health Clinic, at the Miami Vet Center offices in Doral and its outstation in Monroe County, as needed.
    • Facilitate bi-weekly supervision sessions between Outreach Specialists, Case Managers, and Housing Specialists for the purpose of assessing housing progress while maintaining the positive mental health of our staff and guarding against secondary symptoms of PTSD.

    KNOWLEDGE, SKILLS, AND ABILITIES

    • Extensive knowledge of existing community resources for veterans and their families in Miami Dade Broward, and Monroe Counties.
    • Ability to design and manage a new human services program through its startup activities and development of business relationships and support.
    • The ability to establish and maintain referral networks through community outreach.
    • Strong interpersonal skills in order to interact with a wide variety of health and human service providers involved in service delivery to veterans, active duty personnel, and their families.
    • Commitment to multidisciplinary collaboration, communication and consensus-building to improve access and minimize duplication and fragmentation of efforts to serve veterans and their family members
    • A high level of supervisory and administrative experience in private and/or the not for profit sector.
    • Strong analytical skills in order to effectively understand, interpret, and translate data into meaningful information and use toward accomplishing the goals and objectives of the Advocate Program.
    • Excellent verbal and communications skills.
    • Excellent grant writing abilities.

    EDUCATION and/or EXPERIENCE

    Must have excellent interpersonal skills coupled with excellent oral and written communication skills. Must possess strong observation and analytical skills; functional knowledge of emergency services; strong personal responsibility. Bachelor’s degree required, Master’s degree preferred. Current training on military family benefits and familiarity with military family challenges is preferred. Flexible approach to working hours is mandatory, including after hours for emergencies when applicable.

    LANGUAGE SKILLS

    Ability to read, analyze and interpret program procedures, outreach protocols and related policies. Ability to write reports, correspondence, and assist team in outreach tools development. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public. Bilingual preferred(Spanish/English, Haitian Creole/English)

    TECHNICALSKILLS

    Proficient in Microsoft Office 365 program, Salesforce, Homeless Management Information System (HMIS)

    REASONING ABILITY

    Ability to define problems, collect data, establish facts, and draw valid conclusions.

    CERTIFICATES, LICENSES, REGISTRATIONS

    Must possess valid Florida driver’s license.
