Program Manager (Non-Profit) manages a team that coordinates the programs of a non-profit organization. Designs and implements programs that support the organization's mission, vision, and goals. The Program Manager (Non-Profit) manages subordinate staff in the day-to-day performance of their jobs. True first level manager. Prevention Care Manager

    Provides linkage and navigation services to participants within STAP/STCC harm reduction programs. Provides referrals, harm reduction education, and outcome follow-up for participating clients. Coordinates care and transportation to increase access to medical and behavioral health care, harm reduction education, testing services, as well as social service benefits & financial entitlements. Responsible for providing services to participants in outlying areas. Offers low threshold care coordination services, as well as completing intake, risk assessment, and HIV/HCV testing services.

     

    ORGANIZATIONAL RELATIONSHIP

    Reports directly to the Outreach Coordinator.  Collaborates with other Prevention Services staff members, STAP staff members, and volunteers/interns as appropriate.

    SPECIFIC RESPONSIBILITIES

    1. Maintains strict confidentiality in all aspects of STAP activities.
    2. Primary responsibilities include provision of linkage and navigation services to eligible individuals. This includes but is not limited to: engaging clients in the performance of: initial intake, risk assessment, action plan development and implementation, follow-up contacts;, case conferencing with other agencies/service providers/medical staff, in order to ensure comprehensive care and promotion of improved medical outcomes and connections to care in the community;, assisting with obtaining needed linkages and referrals, including: financial, transportation, housing, Primary Care and other medical/behavioral health services as appropriate;, monitoring referral outcomes and verifying services with external providers, serving as client advocate as necessary; , assisting with crisis intervention activities for clients; and, maintaining working relationships with social/financial assistance providers, community service providers, and regional Medicaid providers.
    3. Responsible for all duties related to general SEP operations and programming.: Assists with staffing the SEP during operation hours (M, T, TH, F 12-5pm) . Performs initial intake of syringe exchange participants and referrals for additional needed services, Provides comprehensive risk assessment services, as well as short-term prevention care management services to eligible individuals , Coordinates the exchange of used syringes returned by participants for new syringes and maintains the exchange transaction logs, Conducts exchange related duties throughout 10 county region using agency vehicles, including mobile syringe exchange unit, Offers complete prevention protocol consisting of safer injection education and supplies, safer sex education and supplies, Conducts needs assessment and education related to risk reduction, treatment options, and community referrals, Maintains all written and computer data of daily transactions, Maintains the cleanliness and proper functioning of site and equipment, Maintains the scheduled hours of site., Works collaboratively with other STAP personnel who assist in staffing
    4. Inputs AIRS data as directed by the Harm Reduction Coordinator.
    5. Responsible for safety in the workplace; must follow safety, healthy and security policies, rules and procedures. Report, and if possible, correct unsafe conditions observed in the workplace. Ensure all accidents/injuries are reported following the agency's policy and procedure.
    6. Serves as an agency and/or client liaison/advocate with health and human service providers within the continuum of care and services needed by the clients. Provides linkages, referral, advocacy, and assistance with information including but not limited to: HIV and HCV disease, harm reduction, benefits related to treatment adherence, treatment options, benefits/entitlements, HIV/Hepatitis C disease, etc. As such, care managers are responsible for establishing contacts and relationships with individuals and providers in their designated area of responsibility.


    1. Stays current on developments regarding HIV/AIDS, Viral Hepatitis, and Sexually Transmitted Infections and other health issues as well as the “person-centered care” model of service delivery. This includes but is not limited to specialty medical services, medication management, motivational interviewing techniques, current research on behavior change, HIV/HCV (prevention, transmission, risk reduction), related diseases, HIV confidentiality law, infection control, etc.
    2. Coordinates daily activity with the Harm Reduction Coordinator or designee.
    3. Participates in required training and seminars to maintain knowledge of current developments in HIV/AIDS and Harm Reduction service delivery.
    4. In accordance with STAP/STCC’s data collection procedures, retains accurate records of all case management activities (e.g. intakes, assessment, progress notes, contacts, crisis intervention, reassessment, service plan, etc.), and outreach activities.
    5. Assists in preparation of the monthly, quarterly, and year-end reports.
    6. Participates in regular STAP staff meetings, keeping colleagues informed on client services and maintaining knowledge of other STAP and STCC programs.
    7. Related duties as assigned.
