    The Youth Case Manager is responsible for case management, coordinating programming, services and support for youth in the assigned geographic service area, with the goal of promoting protective factors and college, career, and employment readiness.

    RESPONSIBILITIES:

    1. Initiate contact, establish rapport, and communicate effectively with at-risk youth in low-income neighborhoods
    2. Support the Program Director in engaging appropriate community resources (e.g. coordinating with organizations, programs, leaders, volunteers, and participating youth)
    3. Recruit 50-100 youth to participate in activities which contribute community change (e.g. researching youth violence issues and developing/advocating for community-based solutions)
    4. Facilitate a team of core youth participants to conduct this work using a structured, evidence-based youth organizing process 
    5. Provide facilitative leadership experience, interactive group methods, and youth organizing including major event planning/coordinating and activity planning/coordinating.
    6. Guide/support/train youth to facilitate meetings/discussions, prepare and deliver presentations, and participate in productive dialog with adult community stakeholders. This may include preparing youth to advocate effectively for changes in policies/practices/resources affecting youth and community.
    7. Case manage referral youth and center walk in youth. Coordinate with site case managers to provide wraparound family services.
    8. Support the college/career readiness component by discussing college/career readiness steps and progress with participants including job readiness and application process. Assist with mentor/volunteer recruitment.

     REQUIRED KNOWLEDGE, SKILLS and ABILITY:

    • Working knowledge of challenges and barriers faced by at-risk youth (psychological, logistical, financial, legal, stigmatization, family-related, peer-related, etc.)
    • Ability to work with at-risk youth and families from multiple ethnic backgrounds.
    • Ability to conduct outreach including door-to-door, parks, home visits, etc.
    • Ability to learn the neighborhood-based Family Resource Center (FRC) approach to providing social connections, positive peer associations, and community support and the Positive Youth Development Model for youth organizing.
    • Ability to learn and incorporate the principles of family support and protective factors.
    • Knowledge of community resources within local area.
    • Involvement with grassroots organizations, resident organizations, and neighborhood groups.
    • Ability to work with mentors who will also be working with the youth
    • Must adhere to all CPFSJ policies, partner agreements (MOUs, LOAs, Contracts, etc..), federal, state, and local laws and regulations.
    • Must maintain Youth Mental Health First Aid Certificate.

    EDUCATION / EXPERIENCE:

    College and/or equivalent proven work experience in social work, sociology, psychology or related area working in a community based or non profit organization or providing case management to families in non-profit field.  The Agency invites applicants with non-traditional career development tracks.

     REQUIREMENTS:

    • Must be authorized to work in United States.
    • Must provide own transportation and have a valid driver’s license and provide proof of insurance.
    • Pass a background check

     PHYSICAL REQUIEMENTS:

     Employee in this position must have the ability to:

    • Sit for extended periods of time.
    • Stand and walk for a long period of time, bend over, reach overhead, grasp, push, pull and move, lift and/or carry at least 30 pounds to waist height. 
    • Enter data into a computer terminal/typewriter, operate standard office equipment, and use a telephone
    • See and read a computer screen and printed matter with or without vision aids
    • Hear and understand speed at normal levels and on the telephone.
    • Employees in this position will be required to work indoors in a standard office environment, and come into direct contact with CPF’s staff, other agencies and the public.
    • Employees in this position will also be required to outreach to target neighborhoods and schools, and work outside of the office atmosphere. 
    • Must pass background check
    • Must provide own transportation and have a valid driver’s license and provide proof of insurance.

    DESIRABLE

    Bilingual In one of the following languages: Spanish. Pashto. Urdu. Hmong. Vietnamese. Cambodian, or Laotian

    COMPENSATION:

    Starting Pay Scale: $17.00 to $19.00 per hour, based on experience.  Full-time, non-exempt position subject to overtime and comprehensive benefits including medical, dental, vision, life and retirement.  

    Monday through Friday, 8:30 a.m. to 5:30 p.m. or 8:00 a.m. to 5:00 p.m.
