Program Manager (Non-Profit) manages a team that coordinates the programs of a non-profit organization. Designs and implements programs that support the organization's mission, vision, and goals. Being a Program Manager (Non-Profit) monitors program operations for quality, efficiency and effectiveness. Utilizes a client relationship management (CRM) or another system to organize information and facilitate outreach communications, perform forecasting, and measure the performance of programs. Additionally, Program Manager (Non-Profit) implements a fundraising and development strategy to identify potential donors or funding sources and conduct outreach initiatives. Collaborates with partners to ensure that special events publicize the organization and its programs to the community and support its goals. May assist in the development of grant applications or proposals. Requires a bachelor's degree. Typically reports to a director. The Program Manager (Non-Profit) manages subordinate staff in the day-to-day performance of their jobs. True first level manager. Ensures that project/department milestones/goals are met and adhering to approved budgets. Has full authority for personnel actions. To be a Program Manager (Non-Profit) typically requires 5 years experience in the related area as an individual contributor. 1-3 years supervisory experience may be required.     Primary Role:

    Focus on the care coordination and management of patients with chronic or infectious diseases.


    Qualifications:

    • HS Diploma or equivalent required
    • Medical Assistant certification or 3 years’ experience working in primary care setting required
    • Knowledge of chronic illnesses and other health disparities that impact the Black/African and Latino American community
    • Sensitivity to populations at increased risk for hospitalizations
    • Sensitivity to ethnicity, culture, gender, sexual orientation, values, beliefs, and behaviors
    • Strong organizational skills and attention to detail
    • Excellent written and oral communication skills
    • Ability to multitask in a fast-paced work environment
    • Positive, professional, team-oriented attitude
    • Proficient in Microsoft Word and Excel
    • Travel Requirements: This position may require travel between CCHN sites


    Essential Job Duties:

    Works in collaboration and continuous partnership with chronically ill or “high-risk” patients and their family/caregiver(s), clinic/hospital/specialty providers and staff, and community resources in a team approach to:

    • Promote a culture of wellness.
    • Promote timely access to appropriate care.
    • Increase utilization of preventative care
    • Reduce emergency room utilization and hospital readmissions.
    • Increase comprehension through culturally and linguistically appropriate education.
    • Create and promote adherence to a care plan, developed in coordination with the patient,
    • primary care provider, and family/caregiver(s)
    • Increase continuity of care by managing relationships with tertiary care providers,
    • transitions-in-care, and referrals
    • Increase patients’ ability for self-management and shared decision-making.
    • Provide medication reconciliation.
    • Connect patients to relevant community resources, with the goal of enhancing patient.
    • health and well-being, increasing patient satisfaction, and reducing health care costs.
