Key Duties
- Provide case management services to individuals and families in need
- Conduct comprehensive assessments to identify clients' needs and develop appropriate service plans
- Collaborate with clients, service providers, and community resources to coordinate and monitor services
- Advocate for clients' rights and access to necessary services
- Provide crisis intervention and support during emergencies or difficult situations
- Maintain accurate and up-to-date case records and documentation
- Monitor client progress and evaluate the effectiveness of interventions
- Provide referrals for additional services as needed
Experience (starting wage based on education and experience)
- Education – H.S. Diploma, GED, or College Degree (2 or 4 yr); preferably in related field of study
- Experience -- Minimum two years direct paid experience in social services, preferably with youth and/or families or closely related field
- Community health Worker (CHW) or Traditional Health Worker (THW) ready, in progress, or obtained; or Qualified Mental Health Associate (QMHA-I) ready or obtained
OR
- Any combination of education and/or experience/certifications above
Required Skills
- Strong professional resilience
- Strong work ethic and professional judgement
- Able to complete required data and case notes on-time with accuracy
- Knowledge of social services resources & programs a plus
- Strong interpersonal skills and the ability to relationship build with diverse populations
- Excellent communication skills, both written and verbal
- Ability to work independently with minimal daily direction
- Contributing team member
- Strong organizational and time management skills
- Ability to handle confidential information with sensitivity and professionalism
- Knowledge of youth and family evidence based best practices
