Job Title: Provider Assistant / Medical Assistant

GENERAL PURPOSE:
The Provider Assistant is responsible for integrating both clerical and clinical tasks to enhance workflow by assisting the designated SNP provider/s provide continuity of care in both the long-term and post-acute rehabilitation setting.
QUALIFICATIONS

- Certification as a Medical Assistant is preferred.
- Knowledge of long-term care environment is desired.
- Must possess the ability to communicate effectively and respectfully with NP practice members, facility staff, patients, the medical community, and other customers.
- Must possess computer expertise as well as a knowledge of electronic medical record documentation.
- Capable of autonomous documentation requirements.
- Must have patience and a positive attitude toward the elderly and disabled.
- Must be able to speak, read, write, and understand English.

STANDARD REQUIREMENTS

- Report to management immediately all instances of misappropriation/handling of resident property.
- Support, cooperate with, and implement specific procedures and programs for:
- Safety, safe work practices, established fire/safety/disaster plans, risk management, security, report and/or correct unsafe working conditions, equipment repair and maintenance needs.
- Maintain confidentiality of all data, including resident, employee, and operations data.
- Comply with all regulatory requirements.
- Comply with current law and policy to provide a work environment free from sexual harassment, and all illegal and discriminatory behavior.
- Support and participate in common teamwork.
- Cooperate and work together with all co-workers.

- Comply with all company policies and procedures regarding HIPAA Privacy and Security, the company’s Corporate Compliance and Quality Assurance program requirements, including the Code of Conduct, and neglect state mandated requirements in accordance with Federal requirements.
DOCUMENTATION
- Must comply with outlined job functions and readily adapt to a changing environment.
- Must complete documentation requirements efficiently.
- Must document in multiple electronic medical record systems and update each EMR as outlined in job duties.
ESSENTIAL JOB FUNCTIONS
- Receive patient list daily from designated provider(s) via secured NHS email for the purposes of documentation and reconciliation.
- Maintain a template spreadsheet for all designated provider(s) for Simpra Advantage members for metrics tracking purposes as outlined in training course.
- Copy and paste provider clinical notes from EHR systems and update clinically
necessary metrics.
- Monthly medication review between EHR systems to ensure consistent documentation
and update EHR system when appropriate and ensure metrics are completed as required.
- Interact quarterly with care plans as directed by designed provider(s).
- Review HCC/ICD10 codes quarterly and communicate inconsistencies to the designated
Provider(s) for review.
- Coordinate MTM (medication therapy management) annual reviews between the consultant pharmacist and the designated provider(s)
- Other duties determined to streamline workflow processes for SNP providers and staff.
OTHER JOB FUNCTIONS
- Performs all other duties as assigned by Office Manager.

- May be subject to frustrations in meeting work demands due to frequent interruptions.
- May be exposed to infectious waste; diseases, conditions, etc, including exposure to AIDS and Hepatitis B viruses.
