    Job Summary and Qualifications

    The Plus Care Networks (PCN) Director serves a leader in aligning HCA Healthcare’s key strategies with the complex PAC (Post-Acute Care) continuum. The ideal candidate functions as a key resource and subject matter expert for all PAC related initiatives, challenges, and collaborative efforts to improve care transitions and patient outcomes.

    The PCN Director works with hospital executive and clinical leaders to develop and maintain an ongoing collaborative relationship with a group of PAC facilities and agencies selected to participate in a post-acute network known as the Plus Care Network (PCN). These relationships are managed with mutual respect and seek high quality outcomes and patient satisfaction. The PCN Director works to continually shape the Network landscape by offering objective data and education to all stakeholders in the market.

    PCN Directors will support up to six, but typically five, hospitals and their selected PCN providers. Each hospital selects 10 to 15 PAC providers across various levels of care, including Long Term Acute Care, Inpatient Rehabilitation, Skilled Nursing, Home Health Care, and Hospice. Therefore, the PCN Director must be capable of managing a large and complex network ranging from 60 to 75 PAC providers. In addition, the PCN Director must work diligently to ensure all regulatory and compliance requirements are maintained in accordance with company policy.

    The PCN Director plays a vital role in achieving positive patient outcomes in the market including but not limited to addressing barriers to acute throughput, improving Skilled Nursing Facility (SNF) length of stay (LOS), increasing the efficiency of PCN facilities and agencies delivery of care, increasing discharges to PCN providers, monitoring of readmission trends, tracking quality of care measures across all PCN providers, and educating hospital and PCN leaders on strategies to improve performance.

    Duties and Responsibilities:

    • Responsible for developing a network of PCN facilities and agencies for a defined list of hospitals. Our targeted ratio is one PCN Director for five hospitals/networks.
    • Ensures all necessary logistics obtained to support a successful PCN (contacts, EMR access, onsite access, etc.).
    • Responsible for maintaining positive relationships with PCN providers and hospital staff to support the patient centric model in a highly collaborative environment.
    • Provides regular onsite and remote reviews of PCN provider operations to ensure quality patient care and staff support and communicates results to hospital and PCN leadership.
    • Executes department goals by leading initiatives, meeting deadlines, and coordinating resources between hospitals and PCN providers to ensure efficient and effective operations.
    • Gathers and monitors relevant data regarding all post-acute facilities and agencies in the community and the changing dynamics within the market.
    • Monitors publicly reported data of post-acute facility and agency metrics (e.g. readmission rates, ALOS, quality metrics, patient experience, etc.).
    • Identifies challenging situations early and ensures appropriate action taken to mitigate escalation, works to resolve PCN provider complaints, and leads issue resolution.

    Requirements:

    • Experience in Acute and Post-Acute Care Continuum
    • Demonstrated ability to independently plan, organize and prioritize work
    • Demonstrated willingness to work toward team objectives
    • Executive communication and presentation skills
    • Proficiency with Microsoft Office applications, specifically Excel and PowerPoint
    • Data analytics and reporting experience
    • Experienced in Value Based Care models, managed care, and provider relations
    • Effective negotiation and influencing skills
    • Ability to build and foster relationships inside and outside the organization
    • Must reside in the Nashville area

    Experience/Education/Certificate(s)/License(s):

    • 3-5 years of working experience in acute and post-acute care continuum
    • 3-5 years in a customer facing or Account Management Role
    • Bachelor’s Degree in therapy, nursing, social work, or healthcare related field required(consideration may be given to candidates with equivalent relevant work experience)
