PROVIDER NETWORK MANAGER manages the operations of a healthcare provider network. Responsible for establishing and maintaining processes and systems to provide routine services to members including contract management and credentialing. Qualifications:
Bachelor’s Degree Required. Valid PA Driver’s License, eligibility to work with children under PA legal and regulatory requirements. State Police, FBI and Child abuse clearances required. Experience working with children is helpful.
Responsibilities:
The Case Manager will work to support the mission statement of Families United Network Inc. and perform work requirements in a manner that will further the agency’s purpose and impact.
The Case Manager will be sensitive to problems and case issues, utilizing community resources and treatment team members’ expertise to go beyond mere compliance and cooperation to active support, encouragement and teamwork.
A typical day involves meeting with families and children in care to monitor progress in placement status, participating in and providing leadership in activities that promote a safe and enriching environment for children in care, assisting children in navigating their past trauma and helping to build resilience and coping skills, supporting resource families and children in developing healthy relationships, ensuring that transportation is available for children as needed, participating in court and legal meetings with county agencies, and completing accurate and timely written documentation including progress reports, individual service plans and safety assessments.
