Participates in the negotiation of contracts and prepares reports and analysis of contract details and statistics. Being a Provider Network Specialist distributes contract information, status updates, and other information within the organization. Requires a bachelor's degree. Additionally, Provider Network Specialist typically reports to a manager or head of a unit/department. The Provider Network Specialist gains exposure to some of the complex tasks within the job function. Occasionally directed in several aspects of the work.   • SUMMARY: 
    Community Networking services provide individualized day activities that support the participant’s definition of a meaningful day in an integrated community setting, with persons who are not disabled.  This service is provided separate and apart from the participant’s private residence, other residential living arrangement, and /or the home of a service provider.  These services do not take place in licensed facilities and are intended to offer the participant the opportunity to develop meaningful community relationships with non-disabled individuals.  Services are designed to promote maximum participation in community life while developing natural supports within integrated settings.  Community Networking services will enable the participant to increase or maintain their capacity for independence and develop social roles valued by non-disabled members of the community.  
     
    Community Networking services consist of: 
    • Participation in adult education
    • Development of community-based time management skills
    • Community based classes for the development of hobbies, leisure/cultural interests
    • Volunteer work
    • Participation in community group or associations
    • Training and education in self-determination and self-advocacy 
    • Using public transportation
    • For children that need staffing supports to assist them to participate in day care/after school summer program
    • This service includes a combination of training, personal assistance and supports as needed by the participant during activities.  
    • Transportation to/from the participant’s residence and the training site is included.  
      
    Job Responsibilities:
    • Communicate relevant consumer information to Family Service’s Coordinator, care coordinator and or parents
    • Provide privacy and confidentiality of all consumer records
    • Assure consumers are free from abuse, mistreatment and or neglect and report any incidents of such to Family Service’s Coordinator, care coordinator and or parent
    • Treat each consumer with dignity and respect
    • Complete all documentation related to Individual Support Plans
    • Develop and demonstrate a thorough understanding of philosophy and purpose of caring for an MH/DD consumer
    • Develop familiarity with the treatment plan of each consumer served
    • Implement goals and recommendations for client served and follow appropriate documentation procedures
    • Provide a positive atmosphere which facilitates growth and learning
    • Make decisions based on training and policies when unsupervised
    • Perform duties and all other requested duties when requested by supervisor

    Staff Qualifications: 
    • Staff must meet the requirements for paraprofessionals in 10A NCAC 27G.0200
    • Staff must have a high school diploma or GED. 
    • Staff must meet client specific competencies as identified by the individual’s person-centered planning team and documented in the Individual Support Plans.
    • Staff must successfully complete First Aid, CPR and DMH/DD/SAS Core Competencies and required refresher training.
    • Paraprofessionals providing this service must be supervised by a Qualified Professional. Supervision must be provided according to supervision requirements specified in 10A NCAC 27G.0204 and according to licensure or certification requirements of the appropriate discipline.
    • Must have a criminal record check
    • A healthcare registry check as required in accordance with 10A NCAC 27G.0200
    • If providing transportation, have a North Carolina or other valid driver’s license, a safe driving record and an acceptable level of automobile liability insurance.
