JOB SUMMARY: A professional who assists the QAPI Coordinator with the development and ongoing administration of quality assessments and chart reviews.
QUALIFICATIONS:
1. Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing; or
2. A qualified health care professional.
3. Three to five (3-5) years of health care experience, preferred.
4. One (1) year experience in-home care, preferred.
5. Oasis and Coding Certified.
5. Ability to exercise initiative and independent judgment.
6. Ability to work with individuals, enlist the cooperation of many people to perform/achieve a common goal.
RESPONSIBILITIES:
1. Understands and adheres to established Agency policies and procedures.
2. Understands and promotes principles of continuous performance improvement.
3. Assist with the orientation of new staff to the Agency’s QAPI program.
4. Assists in the planning and consultative needs of staff.
5. Assists in the preparation and implementation of policies and procedures which meet Medicare, Medicaid, accrediting bodies, state, and local laws.
6. Participates in studies and research and other administrative functions as assigned.
7. Serves as a role model for all colleagues by setting an example of high standards in dress, conduct, cooperation, and job performance.
8. Observes confidentiality and safeguards all patient-related information.
9. Accepts responsibility for regular attendance and punctuality; fulfills job-related requirements without regard to the time involved.
10. Serves as a resource person to employees.
11. Reports any problem relating to patient care and/or employee well-being.
12. Immediately reports any accident, incident, lost articles, or unusual occurrence to the Administrator/Branch Director.
13. Attends pertinent continuing education programs other than routine in-services and shares information with staff.
14. Assists in the development of process improvement activities with appropriate data collection, aggregation, analysis, taking action, and reporting of results according to the Agency’s QAPI plan.
15. Reviews patient medical records for compliance with federal, state, accrediting bodies, local, and Agency policies and guidelines.
16. Assist in preparing QAPI reports/minutes of meetings; forwards reports according to QAPI plan.
17. Other duties as assigned by the QAPI Coordinator/Branch Director/Administrator.
18. Understanding of ICD10 coding and ability to perform.
WORKING ENVIRONMENT: Works indoors at home and in the home health office when necessary.
