Responsibilities:
- Hold meetings with patients/residents, families, and significant others as requested and to provide support and information so as to improve quality of life
- Establish relationships and maintain contact and referral flow with community based agencies/services that benefit our patient/resident population
- Assist the Quality Assurance Committee in developing and implementing appropriate plans of action to correct identified deficiencies, prepare assigned information for input in to the MDS computer program
Qualifications & Experience Requirements for Social Worker:
- Previous Experience as a Social Worker preferred
- Must have a Valid Social Worker License
- Must have Bachelor's degree from accredited school of Social Work or related field
