    Summary:

    Responsible for various medical records tasks including record organization, record
    accuracy/completeness, scanning, filing, training and record auditing. Collaborates with staff,
    insurance companies, attorneys and other individuals regarding records issues.



    Essential Responsibilities and Target Outcomes:

    • Opens/closes access to client records, purges closed files for storage, thins overflow records out
    of active area, sorts and files forms, reviews clinical records for completeness prior to filing and
    returns incomplete forms to senders for completion. Tracks and reports delinquencies of
    documents necessary for record closing.
    • Extracts medical records for clinical appointments and requests for information, ensuring that
    information is accurate and timely.
    • Files appropriate clinical forms and correspondence in medical records in an accurate and timely
    manner.
    • Prepares records for release by obtaining releases and copying parts or all of the record
    material.
    • Performs record audit related tasks as required by licensing bodies, payers and accreditation
    organizations by reviewing for accuracy and timeliness.
    • Maintains confidentiality and security of medical records and department information according
    to established procedures with no reported errors by ensuring that only authorized individuals
    have access to the medical records room/records.
    • Provides medical records training to clerical support staff who are responsible for maintaining
    medical records.
    • Performs other related duties as assigned.
    Principal Customers:

    Program Managers and staff, client record management and audits, accreditors and license bodies –
    record review/management and compliance issues.

    Critical Work Relationships:
    Same as principal customers.


    Minimum Requirements:

    Education: High School Diploma or GED.

    Experience: • Successful experience in record management/filing or
    equivalent experience in a medical office with records
    responsibilities, preferably a minimum of one year.

    • Experience and/or training with agency accreditation and
    HIPAA required
    • Intermediate experience with Microsoft Office products
    including Word, Outlook and Excel.
    • Experience utilizing client data bases and linked clinical
    business applications, including Electronic Medical Record
    (EMR) systems. Experience with EPIC EMR preferred.

    Competencies:
    Behavioral Skill Sets

    • Ability to apply basic knowledge of the Health Insurance Portability
    and Accountability Act Privacy and Security regulations and
    provisions (HIPAA).
    • Ability to apply knowledge of maintenance of clinical/medical
    records processes.
    • Attention to detail in ensuring completeness and accuracy of client
    data.
    • Ability to work independently by prioritizing multiple tasks/projects;
    ability to apply organized work skills in meeting shifting priorities
    and deadlines.
    • Ability to collaborate with others in working towards resolution of a
    question or an information request.
    • Interpersonal and communication skills in serving requests from co-
    workers and program staff.
    • Ability to ensure the security and confidentiality of client clinical and
    financial information and records in a manner consistent with
    professional and CMHA codes of ethics.
    • Awareness/sensitivity to all types of issues related to client
    diversity.
