POSITION IS LOCATED IN SHELBY, NC
Recreational Therapist/Activities Coordinator Summary of Position: Along with the Recreational Therapy/Activities Coordinator and Center CNAs, the Recreational Therapist/Activities Coordinator (RT/AC) assists with the responsibility of coordinating, planning, and implementing of the therapeutic and or recreational activities for the participants of Senior TLC. Reports to Center Manager.
Recreational Therapist/Activities Coordinator Qualifications:
Education: BS degree in Recreation Therapy, NC Activity Coordinator certification, or High School Graduate with at least 3 years of experience working with the frail elderly population providing activities services.
Experience: Minimum 1 year of experience in a social or recreational program providing and coordinating services for a frail elderly population within the past 5 years.
Licensure, Registry, or Certification Required: Licensed Recreational Therapist in NC or Activity Coordinator certification. MUST have a valid Activities Director/Activities Coordinator certification in the state of NC to be in compliance with our regulations.
Special Training: Must be able to work effectively in a team environment. Good understanding of psychosocial problems of aging. Only act within the scope of his or her authority to practice. Meet a standardized set of competencies for the specific position description established by Senior TLC and approved by CMS before working independently.
Key Responsibilities
1. Participates as a member of the Interdisciplinary Team. Conducts an initial assessment and scheduled reassessment of all participants, minimally every 6 months or with a significant change in condition and assessment must be completed prior to scheduled team conference and/or care planning. Communicates participant changes with team members
2. Records, signs, and dates progress reports in the participant's medical record.
3. Helps to develop, implement, and supervise the Recreational/Activity Program in the center.
4. Plans and provides both group and individual activities.
5. Develops specific programs as part of the plan of care to include therapeutic recreational and/or social activities.
6. Serves as a resource to staff and coordinates volunteer activities at the center.
7. Builds within the Center a sense of community and a warm social atmosphere in which participants are able to identify a support system.
8. Plans a monthly/daily calendar for the Adult Day Health Center based on participant interest and in collaboration with the Center Lead CNAs
9. Participates in Quality Improvement (QI) activities as appropriate
10. Coordinates with various community resources.
11. Supports Senior TLC’s mission to encourage and support the quality of life of seniors wishing to continue living in the community; its vision to be the preferred provider of individualized care for seniors in the community; and its values of respect, integrity, accountability, compatible goals, and compassionate care
12. Other duties as assigned.
