Additionally, Referral Specialist documents referrals, communications, actions, and other data in an information management system. Generates and distributes all applicable forms, notifications, and paperwork. Requires a high school diploma. Typically reports to a supervisor. The Referral Specialist works under moderate supervision. Gaining or has attained full proficiency in a specific area of discipline. - Answering office phone and directed them
- Receive and process incoming referrals for TFC and IL programs
- Utilize OneCall for initial communication with potential families
- Maintain information stored in OneCall
- Call families who have opted out of OneCall
- Contact potential families to facilitate placement
- Maintain ongoing contact with placement referral sources
- Maintain database by entering all referrals and enrolling clients at the of intake
- Update home capacity form
- Update detailed home listing
- Quality Assurance- Surveys for foster parents
- Serves as the initial telephone contact for DCBS referrals
- Generates applicable intake reports
- Refer consumers to other community resources as needed
- Attend and participate in meetings, committees and trainings as assigned
- Provide census data
- Other duties as needed
Qualifications:
A person employed as Referral Specialist shall possess the following qualifications:
- Must have excellent written and oral communication skills, sound judgment, therapeutic skills
with a multi-disciplinary team.
- Ability to maintain positive professional working relationships and to work effectively within a
- Organized, efficient, and able to manage multiple task simultaneously with attention to detail and
- Proficient with Microsoft applications and possess the ability to learn other software
- Knowledge and understanding all agency protocol, policy, procedure, plans programs
