The Case Manager interfaces directly with all members of the multidisciplinary care team, the patient and family/significant others. In addition, the Case Manager interacts with referral sites and community providers at both ends of the continuum of care. He/she analyzes care needs of patients and monitors progress toward desired outcome goals and tracks and analyzes clinical and financial data to support clinical decision-making. Interface occurs with Clinical Information Analysis, Performance Improvement/Quality, and Risk Management Departments, as patient care and facility systems issues are identified. Because of the extensive interface of the Case Manager with the patient and family, he/she must exhibit a thorough knowledge of the principles of growth and developmental needs over the life span of the neonate through geriatric populations.
Current KY RN licensure and CPR required; BSN and minimum of three years in utilization, quality, or case management preferred. Knowledge of acute care regulatory/accreditation requirements, discharge planning experience preferred. Data analysis and interpretation skills. Strong organizational and prioritization skills needed. Highly developed problem solving and deductive reasoning skills. Dependable, self-directed and pleasant. Adaptable and flexible to meet patient/family and department/facility needs. Proven supervisory, leadership, and organizational skills are required. Must obtain and/or maintain current CPR, ACLS, PALS instructor certification.
