Position Description
The Admissions Liaison/Clinical Liaison serves as the primary liaison to prospective patients, families, and designated healthcare providers. This position requires active involvement in program development and facilitates a smooth patient referral/admission. The efforts of this position will work in tandem with the strategic business plan as well as the leadership in the Facility.

Duties and Responsibilities
Administrative Functions
· Receive and follow work schedule or instructions from your supervisor and as outlined in our established policies and procedures.
· Maximize facility census and ensure a high level of resident satisfaction.
· Develop and maintain a good working rapport with the Facility staff.
· Participate in the development and execution of Facility sales and marketing plan.
· Responsible for monitoring and responding on referral platforms.
· Design and develop marketing and promotional materials for the Facility.
· Coordinate special events for the Facility.
· Establish relationships and maintain contact with primary and secondary resources in the health care field.
· Establish relationships and maintain contact with networking and community referral sources.
· Recommend and implement initiatives that manage the target audience's perception of the Facility.
· Maintain data on census.
· Responsible for all census development.
· Act as liaison between the facility and family members during the pre-admission and admission process. Upon admission, refer family members to appropriate Department Head depending on specific need.
· Screen potential patients through on-site assessment.
· Respond promptly to all referrals.
· Determine the patient’s rehabilitation potential, discharge plan, and specialized rehabilitative or equipment needs.
· Complete all required preadmission screening documentation to determine that services are appropriate.
· Assess referrals and make admission decisions based on medical and financial information.
· Communicate to the Facility’s Admissions Department and other pertinent staff all information necessary for admission.
· Provide needed hospital information for new admissions.
· Educate patients, families, and staff regarding the Facility’s record of providing quality care.
· Promote effective public relations with patients, families, healthcare providers, and the community.
· Exhibit professional behavior as a representative of the Facility.
· Must function independently, and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.
· Perform other reasonable duties as assigned by supervisor.
