Rehabilitation Services Manager jobs in Riverside, CA

8 Hour Shifts │8:30AM-5PM│FULL TIME MON-FRIDAY.

We are state-of-the-art skilled nursing and subacute facility featuring 198 beds located in Upland, CA. The primary purpose of your job position is to plan, organize, develop, and direct the overall operation of the Social Service Department in accordance with current federal state, and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Administrator, to assure that the medically related emotional and social needs of the resident are met/ maintained on an individual basis.

Duties:

• Participate in discharge planning; development and implementation of social care plans and resident programs.
• Participate in community planning related to the interest of the facility and the services and needs of the residents and family.
• Assist in arranging transportation to other facilities when necessary.
• Meet with administration, medical and nursing staff, as well as other related departments in planning social service programs and activities.

Requirements:

• Must have SNF experience.

Pay: $24.00 - $28.00 per hour
