Rehabilitation Services Supervisor - Home Care supervises the delivery of occupational therapy, physical therapy, and speech/language pathology programs to patients in a homecare setting. Responsible for providing consultation and monitoring of the performance of the rehabilitation staff. Being a Rehabilitation Services Supervisor - Home Care provides clinical consultation. Requires a bachelor's degree. Additionally, Rehabilitation Services Supervisor - Home Care typically reports to a manager or head of a unit/department. Requires a license in the area of specialization. The Rehabilitation Services Supervisor - Home Care supervises a group of primarily para-professional level staffs. - Pass a Minnesota Department of Human Services Background Study.
- Have A Minnesota Driver’s License, Full Coverage Insurance, and Reliable Vehicle.
- Be willing to transport clients in your vehicle (Clients may have Walkers, Wheelchairs).
- This position involves driving 30 plus miles per day. 2 clients are seen each day. Mon through Friday. 10 clients per caseload.
- Able to document through Case Notes, expediently, capturing details of IHS Scheduled visits with clients weekly.
- Be a team player, attend team meetings and offer ideas and resources.
- Maintain confidentiality with professionals utilizing releases of information when necessary, adhering to all HIPPA regulations.
- Be organized, ethical and trustworthy.
- Technologically, adaptable as mileage, timecards and navigation are all done by apps.
- Complete all training required by State of Minnesota 245D.
- Prefer Covid 19 Vaccinated. Able to provide proof. (Requested by clients we provide services to).
