Rehabilitation Services Supervisor is responsible for the day to day operations of the rehabilitation therapy function. Supervises the delivery of therapy programs to patients. Being a Rehabilitation Services Supervisor responsibilities include providing consultation to and monitoring the performance of the rehabilitation staff. Oversees staff and ensures that all policies and procedures are followed as set forth by the health care facility. Additionally, Rehabilitation Services Supervisor requires a bachelor's degree. Typically reports to a manager or head of a unit/department. Requires a license in the area of specialization. The Rehabilitation Services Supervisor supervises a group of primarily para-professional level staffs. May also be a level above a supervisor within high volume administrative/ production environments. Makes day-to-day decisions within or for a group/small department. Has some authority for personnel actions. To be a Rehabilitation Services Supervisor typically requires 3-5 years experience in the related area as an individual contributor.     Our most important goal is to provide the highest standard of care to our residents in an environment that is safe and secure. You will be responsible for Plan, develop, organize, implement, evaluate, and direct the social service program of this facility. Your focus will be the basic quality-of-life for our residents, so duties such as recording vitals, providing ambulatory assistance, and seeing to hygiene needs will be key in the individualized care.

    Social Service Director's day-to-day will include:

    • Develop and implement policies and procedures for the identification of medically related social and emotional needs of the resident.
    • Participate in community planning related to the interests of the facility and the services and needs of the resident and family.
    • Participate in discharge planning, development, and implementation of social care plans and resident assessments.
    • Exhibit excellent customer service and a positive attitude towards patients
    • Assist in the evacuation of patients
    • Interview resident/families as necessary and in a private setting.
    • Refer resident/families to appropriate social service agencies when the facility does not provide the services or needs of the resident.
    • Provide information to resident/families as to medicare/medicaid, and other financial assistance programs available to the resident.
    • Maintain quality working relationship with the medical profession and other health related facilities and organizations.
    • Interpret social, psychological, and emotional needs of the resident/family to the medical staff, attending physician, and other resident care team members.
    • Participate in discharge planning and make such plans available to the designated party.
    • Demonstrate dependable, regular attendance
    • Concentrate and use reasoning skills and good judgment
    • Communicate and function productively on an interdisciplinary team
    • Sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours

    What we are looking for:

    • Strong interpersonal skills, especially when it comes to delivering sensitive information to patients and/or families.
    • 2 years social service experience
    • Must be alert and attentive to detail.
    • High School Diploma or equivalent required.
    • Possess Social Service certificate or license.
    • Possess leadership and supervisory ability and the willingness to work harmoniously with other personnel.
    • Be able to make independent decisions concerning above job duties.
    • Possess the ability to deal tactfully with personnel, residents, family member, visitors, government agencies/personnel, vendors, and the general public.
    • Be knowledgeable of activity practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
