Renal Dialysis Director directs the staff and programs of the renal dialysis department. Organizes and coordinates inpatient and outpatient dialysis services and operations. Being a Renal Dialysis Director develops and introduces approved standards and guidelines for dialysis operations and programs. Monitors quality of care, compliance, and data collection and leads quality improvement initiatives. Additionally, Renal Dialysis Director oversees the professional development of staff and educational support programs for patients. Requires a bachelor's degree. Typically reports to senior management. The Renal Dialysis Director manages a departmental sub-function within a broader departmental function. Creates functional strategies and specific objectives for the sub-function and develops budgets/policies/procedures to support the functional infrastructure. Concerto is seeking a PRN Social Worker to join its team! The PRN Social Worker provides psychosocial evaluation and counseling patients who reside in a Skilled Nursing Facility or rehabilitation center regarding dialysis related issues; works in collaboration with the multidisciplinary treatment team to incorporate psychosocial needs/concerns in planning care; documents on patients in dialysis chart in keeping with facility policies to meet Medicare and Quality Assessment Performance Improvement (QAPI) requirements. This position requires facilitation as needed between the dialysis unit and the Social Services Department of the SNF. Function as a member of the Interdisciplinary team. Reports directly to the Manager of Social Services.
