Job Summary/Objective:
Oversee the daily staffing needs, supervision of staff, and oversight of the home environment. Provide general education and consistent coaching to Direct Support Professionals (DSPs) about the program, Person-Centered Support Plans, policies, and procedures. Provide personal care and supervision of persons supported to assure that their daily needs are met. Assist those who need support to lead self-directed lives and to participate fully in their community. Honor the individual path suggested by the unique gifts, preferences, and needs of each person supported. Partner with persons supported and those who are significant to them to lead a life of contribution, freedom, opportunity, and well-being.
Job Description/Responsibilities:
· Participate in persons supported planning & Person-Centered Support Plan meetings as needed
· Collaborate with Residential Case Manager to create staff schedules & find substitute staff for planned/unplanned staff absences
· Collaborate with Residential Case Manager & Coordinator of Health Support Services to arrange medical care appointments, as well as transport & accompany persons supported to these appointments as needed
· Arrange and/or provide transportation for persons supported to employment, day services, and recreational activities within the community
· Participate in planning & facilitating community and recreation activities
· Plan, purchase, and prepare nutritionally balanced meals with input & assistance from persons supported
· Assist persons supported to shop for needed personal & other items
· Document purchases & expenditures to maintain accurate records
· Manage adequate supplies in the home by maintaining supply list, obtaining supplies from the office, and keeping homes stocked
· Train & coach staff regarding their housekeeping responsibilities, as well as assist persons supported with housekeeping and taking pride in their home
· Train & coach staff to complete personal care and other supports & services for residents, as well as assist persons supported with personal care and taking pride in their health
· Maintain knowledge of all competency-based plans to fulfill role of designated trainer
· Consistent availability to DSPs to assist with direct support needs of persons supported, cover shifts, and to assist with emergencies, all as needed
· Maintain adequate supply of prescribed medications available in the home & assist with medication administration when needed
· Maintain the home files & update with current information
· Review documentation/notes for completion on daily basis
· Continually assess staff’s ability to perform supports
· Provide input for training topics for staff meetings
Essential Functions:
· Provide person-centered supports by implementing all of the persons supported plans in a collaborative & expeditious manner through a range of participatory planning techniques
· Promote physical health & wellness by providing personal care
· Practice integrity & responsibility by reporting immediately any person supported incident on the proper forms and to the proper persons as outlined in the Reportable Event Management Policy & Procedures
· Identify crises & use knowledge about crisis prevention, intervention, and resolution techniques while complying with regulations for reporting
· Ensure confidentiality by respecting the privacy of privileged information of persons supported
· Exercise equity, fairness, and justice for persons supported and the community
· Demonstrate respect by fostering pride in the homes of our persons supported by assisting with household operations, such as housekeeping & meal preparation
· Recognize importance of friends, family and community relationships while promoting community living skills to build/maintain relationships by matching specific supports & interventions to unique needs
· Assist with community & service networking by accompanying persons supported as necessary to access resources and to participate in community activities (appointments, community recreation/leisure resources, shopping, etc.)
· Empower self-determination, self-esteem and assertiveness to make own decisions
· Adhere to detailed schedule of support that reflect persons supported goals, outcomes and objectives
· Use effective advocacy strategies to overcome challenges by mobilizing resources to assist reaching goals (including but not limited to education, vocation, career support, etc.)
· Foster professional & timely communication between persons supported, families, DSPs, Case Manager and administration to establish collaborative relationships
· Prioritize organizational participation by attending mandatory agency trainings and meetings to ensure quality practices and supports
· Complete & maintain accurate and timely records for all required documentation (reports, medication administration records, daily notes, progress notes, attendance logs, etc.)
· Respond to needs, desires and interests through knowledgeable formal & informal assessment practices
· Encourage self-development by identifying areas for self-improvement, pursuing necessary educational/training resources, and sharing knowledge with others

Required Education/Certification:
· Proof of High School Diploma, GED or equivalent
o One year of experience preferred in the social services setting.
o CPR/First Aid Certificate (training available upon hiring)
o Medication Administration Certificate (training available upon hiring)
