The Service Coordinator is responsible for the assisting the Adult Program Coordinator with the development, implementation and monitoring of programming for individuals. The Service Coordinator is responsible for the assisting the Adult Program Coordinator with the coordination and facilitation of Person Centered Individual Support Plan process as well as training of direct care staff in the provision of services derived from these plans. Bachelor’s Degree (BA/BS) in Human Services, Education, Special Education and/or related field preferred, high school diploma equired. Experience in teaching with an emphasis on individualized instruction or a background in Social Services. Experience with MR/DD is helpful Must not have any evidence of abuse or fraud in any setting, been convicted of a violent crime or crime related to a dependent population or have repeated or substantial violations of applicable laws and rules in operation of any type of residential, health or developmental program in the care of dependent person. Must have a safe driving record and hold a valid driver’s license and proof of auto insurance.
