Summary:
The Resource Coordinator is responsible for participating in outreach and enrollment, and assistance/ guidance to individuals, family members and caregivers on what services are available to them. The Resource Coordinator serves on an inter-disciplinary team that provides service coordination, managing the medical, social, and behavioral health needs of enrolled members. The Resource Coordinator is responsible for overseeing and facilitating access to all of the services necessary for an individual/family to stay healthy, prevent hospitalizations and maintain stability.
