Respiratory Therapist - Home Care jobs in San Antonio, TX

Prime Home Health is expanding and seeking several compassionate and skilled Physical Therapists, working on a PRN basis, who will play a crucial role in providing comprehensive rehabilitative care to individuals in their Stockdale, Floresville, Falls City, Karnes City, Kennedy, Nixon, Three Rivers, McQueeny homes. You will work closely with patients, their families, and a multidisciplinary team to develop personalized treatment plans and help patients regain functional independence and improve their overall quality of life.

Key Responsibilities:

Qualifications:

• Bachelor's or Master's or DPT degree in Physical Therapy from an accredited institution.
• Current state licensure or eligibility to practice as a Physical Therapist.
• Proficiency in documenting patient information accurately and maintaining electronic health records (WellSky/Kinnser experience a plus).
• Valid CPR BLS certification
• Minimum one year experience in a home health or related setting.
• Strong knowledge of anatomy, physiology, kinesiology, and therapeutic techniques.
• Excellent communication skills with patients, families, and other healthcare professionals.
• Ability to work independently, manage time efficiently, and prioritize tasks effectively.
