• Collaborate with appropriate leadership and staff to develop health awareness programs and events that are evidence-based and supportive of individual or community health needs and concerns.
• Work with other departments and subject matter experts in developing curriculum including needs assessments, program materials and promotion.
• Facilitate a variety of health education and promotion services including presentations, education and skills-based workshops, small group discussions, and other educational sessions or programs in accordance with patient and community needs.
• Create education materials on health-related topics for patient and provider consumption by gathering, compiling, and synthesizing evidence and research-based information.
• Collaborate with other departments within the organization and outside community agencies to create prevention services and educational events.
• Act as a resource for education on health-related topics and provide appropriate evidence-based information to patients, community partners, and community members.
• Assist in the design and analysis of quantitative and qualitative research to better identify patient and community needs and barriers. This includes assisting in survey development, focus group design and facilitation, data analysis and interpretation, and reporting.
• Monitor and evaluate health promotion strategies, working to ensure effectiveness of programming and curriculum design.
• Using evidence-based processes, such as Motivational Interviewing, SMART Goals, and behavior change theory, meet with patients as needed to foster positive behavior change resulting in better health outcomes.
• Demonstrate cultural competency and inclusivity in all aspects of health promotion.
• Appropriately chart all individual patient interactions, generating a care plan summary, and maintain HIPPA compliance.
• Performs other duties as identified or assigned.
