The DeKalb County Health Department is seeking a full-time (37.5 hours per week) Health Promotion Specialist/HIV Case Manager. This position reports directly to the Communicable Disease Coordinator. This position provides case management and coordinates client care to individuals enrolled in Ryan White Part B Case Management services.

Responsibilities

• Responsible for the case management of a caseload of individuals living with HIV/AIDS that includes completing client assessments to determine eligibility for services as defined by grant requirements, completing biopsychosocial assessments on every client to determine appropriate level of services needed and developing medical case management care plans when deemed appropriate.
• Ensures all client services/interactions are documented in an accurate and timely manner in the client tracking system identified by the Illinois Department of Public Health (IDPH).
• Oversees and coordinates the continued care of clinical clients, ensuring effective and high-quality care that addresses specific and personalized needs.
• Provides clients with education and counseling related to HIV and other sexually transmitted infections; transmission risks; HIV / AIDS literacy; sexual health literacy; and risk-reduction practices.
• Refers and links clients to appropriate services within the system of care that promote positive health outcomes, treatment adherence, and greater self-sufficiency.
• Assist clients with overcoming identified barriers to medication and treatment adherence including, but not limited to, housing instability, inadequate nutrition, transportation barriers, substance use disorders and mental health needs.
• Develop meaningful rapport and advocate for clients served by maintaining direct communication with community-based organizations and medical providers.
• Provides crisis intervention and trauma informed care to all clients.

Requirements

• Master of Social Work degree or Licensed Registered Nurse.
• Proficiency in Microsoft Office Suite and ability to work in various web-based platforms.
• Demonstrated skills in critical thinking, time management, excellent written and verbal communication, problem solving/analysis, crisis prevention and intervention.
• Requires practice of professional standards with regard to patient confidentiality.
• May require some early evening and weekend hours.
• Valid Driver’s license and current vehicle insurance coverage.
• Bilingual English/Spanish preferred.
