Scrum Master, Sr. coordinates a cross-functional Agile development team utilizing the principles and tools of the Scrum Framework. Assumes a servant leadership role to support a self-organized team to complete Scrum projects. Being a Scrum Master, Sr. identifies and removes impediments to progress. Facilitates sprint planning meetings, daily stand-ups, sprint reviews, demos, and retrospectives. Additionally, Scrum Master, Sr. identifies and manages the backlog. Supports, coaches and mentors team members. Tracks daily sprint progress and maintain schedules. Requires a bachelor's degree. Typically requires Certified Scrum Master (CSM). Typically reports to a director. The Scrum Master, Sr. work is generally independent and collaborative in nature. Contributes to moderately complex aspects of a project.     Description

    SUMMARY: Provides mental health services as defined by CA State Department of Mental Health, including but not limited to: case management for residents including planning, services, monitoring, coordinating with other agencies and other activities as required.     ESSENTIAL DUTIES AND RESPONSIBILITIES: Must be able to perform each essential job function satisfactorily.

    Direct Service and Case Management

    1. In a culturally competent manner will provide services to all residents without regard to race, color, creed, national origin, marital status, sexual orientation, age, sex, religion, and handicap or payment status.
    2. Provide residents with counseling and training; assistance with daily living skills and managing psychiatric symptoms.
    3. Assist in identifying and accessing medical, educational, prevocational, rehabilitative, social and other community services, specific to residents’ needs, goals, and service plans.
    4. Plan and implement cost effective and efficient service plans in conjunction with other staff, families and other service providers.
    5. Initiate preventative measures and create intervention methods for clients or conditions that pose a threat of danger or crisis.
    6. In conjunction with resident, family and providers prepare Annual Assessments
    7. Inform individuals of their personal rights, responsibilities and Company’s grievance procedures. Ensure services are compatible with residents’ freedom of choice.
    8. Transport residents to appointments, outings, etc. Maintain vehicle inspection logs.
    9. Provide information to clients, families, other agencies and the community regarding health care issues including prevention, early detection and remedial care.
    10. Other duties and responsibilities as assigned.

    Teamwork and Collaboration

    1. Maintain cooperative and professional working relationships and communications.
    2. Give input to management on improving the quality of programs and assists in monitoring program effectiveness and conduct resident satisfaction surveys.

    Reporting and Documentation

    1. Mandated to report any “suspected abuse of dependent adults and elders” directly to law enforcement, Adult Protective Services, Ombudsman, or Community Care Licensing.
    2. Monitor residents’ progress and participation toward service plan goals, collect and report outcome data. Document coordination in progress notes and complete all documentation (E.g. annual assessments, service plans and progress notes) within required timelines.
    3. Keep clear, accurate, and professional records in resident’s files, staff logs, and other required communications.
    4. Meet SCCMH and FSI guidelines for documentation and attend QIC as needed.
    5. Meet individual billing expectations for Units of Service and work collaboratively to meet program requirements.

    Activities (As Designated)

    1. Help organize and conduct group treatment and recovery oriented structural activities.
    2. Give capable residents the opportunity to participate in the planning, preparation, conduct, clean-up and critique of the activities.
    3. Adhere to monthly schedule of planned activities.

    Qualifications


    QUALIFICATION REQUIREMENTS: Knowledge, skill, or ability required to perform the job.

    1. Must be at least 18 years of age.
    2. If required to drive, must be 21 and accepted by the Company’s vehicle insurance.
    3. Cleared by the Department of Justice and FBI prior to commencing work at the facility.
    4. TB clearance within the last 6 months. Pass a health screening.
    5. Maintain a valid First-Aid certification.
    6. May be required to serve on company committees.
    7. Complete all annual training requirements as mandated by Community Care Licensing Division, Title 22 and Front St. policies.
    8. Attend mandated meetings and trainings; fulfill all coursework and training hours required
    9. Work in a safe and acceptable manner, following established safety procedures.

    Education and/or Experience

    B.A. or B.S. in social work or a related field; or two years experience working in mental health.

    Knowledge

    1. Requirements for providing care and supervision of residents.
    2. Knowledge and conformity to all applicable HIPAA, OSHA, state and local laws, rules and regulations including Department of Social Services, Community Care Licensing, Title 22.

    Language Skills

    1. Read, speak, write and understand English well enough to follow and share the regulations, requirements and communications for providing care to residents.
    2. Able to effectively present information and respond to questions.

    Mathematical Skills

    Calculate figures and amounts such as discounts, proportions, and percentages associated with completing the requirements of the position.

    Reasoning Ability

    1. Ability to carry out instructions furnished in written, oral, or diagram form.
    2. Competent in dealing with problems involving several variables in different situations.

    Other Skills and Abilities

    1. Timely complete tasks, meet deadlines, problem solve, and coordinate numerous activities
    2. Adjust tasks in accordance with changing deadlines and priorities.
    3. Adequate computer skills to utilize e-mail, word processing and needed programs.

    Physical Demands: Must be met to successfully perform the essential job functions.

    1. Regularly required to talk, hear, and sit, stand for extended periods of time.
    2. Frequently required to reach with hands and arms, stand, walk, stoop, and may occasionally be required to kneel, crouch, crawl or lift up to 50 pounds.
    3. Be physically, mentally and occupationally capable of performing assigned tasks per Title 22 and other applicable regulation.

    Work Environments: Employee may encounter while performing essential job functions.

    1. Noise level is usually low to moderate.
    2. Possible contact with hazardous and caustic household cleaning chemicals.
    3. The actions of clients may be unpredictable. Staff may be verbally assaulted and at times may be physically injured
