Early On Service Coordinator – One Year Temporary Position

Start Date:                     2019-2020 School Year

Qualifications:

1. Certification as a School Social Worker
2. Preferred knowledge of Early On programs
3.             Demonstrated knowledge of child development
4. Demonstrated knowledge of evaluation of family needs and priorities
5. Demonstrated competencies on areas of interpersonal relationships, communication, and organization

Performance Responsibilities:

1. Evaluate family needs in the child's home or preferred site
2. Prepare for and participate in IEP/IFSP process
3. Collaborate with parents, teachers, administrators, and other ancillary and agency staff
4. Maintain accurate and timely records
5. Serve as service coordinator to eligible infant/toddlers and their families
6. Demonstrate professional conduct and exhibit professional growth
7. Effectively plan and organize duties
8. Comply with school district policies and procedures
9. Effectively participate in meetings, committees, staffings and other required district and building activities
10. Coordinate services with other community human service agencies
11. Comply with Medicaid billing procedures

Reports to:                                     Supervisor of Birth to Three

Terms of Employment:              In accord with the Master Agreement
