Director of Social Services
The Director of Social Service has the overall accountability to plan, organize, develop and direct the day-to-day operations in the social service department in accordance with current Federal, State, and local standards, guidelines and regulations to meet our policies and procedures as established by the facility and as directed by the Administrator. The Director of Social Service is a member of the Leadership Team.
Essential Functions, Duties and Responsibilities:
The Director of Social Service is responsible for assisting residents with all social and emotional issues relating to admission during placement and all other aspects of the patient’s life.
Responsible for review of referrals and recommendation for patient admission for short-term and long-term care and placement.
Actively develops relationships and strategies for collaboration with hospitals/health systems and home health/hospice and community providers to promote value-based care delivery.
Performs periodic assessments of all residents to determine status, preferences and needs to promote patient-centered care. Participates in the resident care conference meetings to assist in implementation of care plans. Assess’ all new residents to obtain history, establish specific goals, writes progress notes, and completes the appropriate section of the MDS.
Participates in Utilization Review Meeting and Revenue/Collection Meetings.
May arrange transportation to appointments, answer phones and reports on census reports.
Provides leadership and support for the Compliance and Ethical principles within management and practice.
Review department policies and procedures and participate in recommending identified medically related social service and emotional needs of the resident. Assist residents and families with adjustments following initial placement.
Works closely with residents and families to assist with financial status by assisting with Medicaid application and applying for representative payee.
Encourages resident participation in facility activities and participate in family council meetings for resident benefit. Identifies and documents changes in affect, behavioral or personality.
Ensures that patient's grievances/concerns are fully documented, investigated, reported and addressed in accordance with facility policies and procedures and the Federal/State rules and regulations.
Director of Social Service may represent the Facility Grievance Officer of the facility
Assist in preparing residents and families with the implementation of the discharge planning process from the facility and completes discharge home follow up according to the established schedules.
Intervenes in all resident conflict situations as necessary and writes a psychosocial summary. Responsible for admissions of new residents, transfers and discharges to ensure a smooth transition. Is available for family conferences and attends meetings as directed by the Administrator.
Works collaboratively with Leadership Team and other Departments.
Participates in Facility Quality Assessment & Assurance Committee and various committees as required.
Participates in Quality Assurance Improvement Projects impacting the social service needs of residents and practice.
Depending upon the Facility, other responsibilities may be deemed necessary or appropriate as they relate to caring for the patient and an employee of the Nursing Home.
· Verifying all financial status of the potential resident prior to admission by checking the appropriates sites like Ability, Medicaid portal and obtains a Prior Approval (PA) for Managed Insurance, enter NODs and send various notices as required.
Job Requirements:
Preferred qualifications include a minimum of two years full-time social service or equivalent experience with at least one year of leadership or management experience in the long-term care setting. Available for planned meetings beyond the normal business hours if needed.
