Social Services Director Job Specification:

The Social Services Director will be responsible for providing support, counseling, and advocacy to residents and their families, ensuring their social and emotional needs are met. This role requires strong communication skills, the ability to collaborate with interdisciplinary team members, and a commitment to upholding state and federal regulations related to social services in a long-term care setting.

Social Services Director Essential Duties & Responsibilities:

• Provide social services to residents and their families, including counseling, support, and advocacy.
• Conduct assessments to determine residents' social and emotional needs.
• Develop care plans in collaboration with interdisciplinary team members.
• Coordinate discharge planning and community resource referrals.
• Ensure compliance with state and federal regulations related to social services.

Qualifications:

• Bachelor's or Master's degree in Social Work or related field.
• Current Ohio state licensure as a Social Worker.
• Previous experience in a long-term care setting preferred.
