JOB DESCRIPTION

POSITION:               DIRECTOR OF SOCIAL SERVICES

REPORTS TO:         ADMINISTRATOR

RESPONSIBILITY:

Identifies the medically related social and emotional needs of each resident and provides the services to meet each need.

SPECIFIC DUTIES:

1.                  Assists the residents and families with financial needs:

A.                Reviews Resident Bill of Rights on admission, with resident, family and/or responsible party.

B.                 Reviews facility rules and regulations.

C.                 Obtains signatures on review for filing in medical record.

2.                  Participates in interdisciplinary care conferences:

A.                Identifies social and emotional needs.

B.                 Records goals and observations accordingly.

3.                  Participates in Resident Council Meetings.

4.                  Organizes discharge plan to ensure optimal continuity of care to meet post discharge needs of each resident.

5.                  Assists each resident to adjust to the effects of their illness or disability, including assisting them in re-direction of interests and activities if necessary.

6.            The Manager on Duty will do facility rounds, checking each department and getting reports from the nursing supervisors. Duties will include, but are not limited to, facility tours for prospective admissions, assisting with resolution of resident concerns and overall facility operations

7.                  Other duties and responsibilities assigned by the Administrator.

WORK SCHEDULE:

Days; Monday –Friday. May be required to work Evening shift, Night Shift, Holidays, Weekends as needed.

Required to Participate in Manager on Duty Program.

MINIMUM QUALIFICATION REQUIREMENTS:

·         Education:                         MSW preferred but not required 

·         Work Experience:             One year in an LTC facility
