Observe and assess at Social Services Director level, the social and safety needs of residents.
Essential Functions
· Complete forms, reports, evaluations, studies etc. related to social services.
· Will accomplish all the admission processes to include:
o the completion of the admission agreement
o appraise the resident/responsible party/family of the residents rights
o determine financial needs
o do a thorough and comprehensive social assessment which includes a listing of strengths as well as needs
o report any admissions to administration, nursing, dietary, and bookkeeping departments
· Develop a post-discharge plan following the first care plan meeting which will address the following:
o specific needs and resident preference in the event of needed discharge
o post discharge plans for care-giver
o specific preparation for resident and family
o include plan in discharge chart
· Develop a workable quality assurance program for social services and coordinate this activity.
· Retain a listing of and be knowledgeable of, all available resources needed for comprehensive care needs of residents, to include community agencies.
· Process all abuse and grievance complaints and work with local social services agency in resolution of abuse, keeping administrator aware at all times of progress and process. Initiate filing adult protection forms when indicated.
· Update records according to facility policy.
· Maintain periodic content with family and schedule regular individual conferences with residents.
· Responsible for notifying legally responsible parties of the residents of impending quarterly evaluations of each resident’s care plan and request their input or attendence. If the resident’s condition should warrant more frequent evaluations, the Social Services Director will be responsible for the notification.
· Assist with pre-admission screening.
· Participate in all care plan conferences.
· Interpret facility policies and procedures to employees, residents, visitors, government agencies, etc.
· Counsel resident, family members, and staff.
· Purchase necessary department supplies and equipment, as authorized.
· Organize, maintain, and store all department-specific supplies and equipment.
· Help create an atmosphere of optimism, warmth, and interest in resident’s personal and health care needs.
· Communicate with co-workers at all levels to adequately meet the needs of residents.
· Participate in facility committees and audits, as needed.
· Maintain confidentiality of all resident information and facility business.
· Report to supervisor all complaints and grievances made by residents, families, visiting individuals, agencies, etc.
