Sunny Acres Nursing and Rehabilitation Center is looking for a Director of Social Services to provide exemplary leadership in its social services department. Sunny Acres is a 93-bed skilled nursing home, and 12 unit Legacy Care Assisted Living facility, in Chelmsford, Massachusetts. Sunny Acres is a 3rd generation, family-owned facility with uncompromising philosophy of providing the highest level of excellence in resident care.
Qualifications:
· Massachusetts State License is required. LICSW is preferred, Bachelor’s degree and LCSW is required.
· Candidate is required to have a working knowledge of current federal and state (Massachusetts) regulations governing social service practices in a skilled nursing facility.
Duties and Responsibilities: The Director is responsible for the following –
· Complete psychosocial assessment for all residents.
· Complete required assessments in conjunction with state regulations. Assessments include but not limited to: pre-admission assessment, 48-hour meeting, including written summary of event, initial, quarterly and annual MDS assessments.
· Document necessary clinical notes associated with assessments.
· Take a lead role in Care Plan meetings.
· Where indicated, provide in-service training and education to residents and staff regarding Resident Rights, Advanced Directives and Abuse Policy and Procedures.
· Take a lead role in reporting, racking and investigating resident grievances.
· Participate in discharge planning with other members of the multi-disciplinary team.
· Complete discharge summary and follow-up for all discharged residents.
· Take lead role in executing resident satisfaction surveys for long term residents, as well as, discharged short-term residents.
· Work closely with family members either in family meetings, care plan meeting and/or special events.
· Ensure resident hospitalization protocols comply with state regulations.
· Take a lead role in facility’s Quality Assurance Performance Improvement activities.
· Where indicated, coordinate referrals and admissions with Administrator, Director of Nursing Services and Admissions Coordinator.
· Maintain resident confidentiality / treat residents, families and co-workers with respect.
