Social Services Director - Nursing Home in a nursing home environment, directs, establishes, and plans the overall policies and goals for the social services department. Responsible for patient assessments, care planning and helping the patients and families adjust to their new surroundings or occupations. Being a Social Services Director - Nursing Home plans and administers social service programs. Supervises the facility's social workers. Additionally, Social Services Director - Nursing Home assists in development of policies regarding participation in facility planning for health and welfare services. Requires a master's degree. Responsibilities:
Responsible for developing, planning, implementing, and evaluating social service programs and services in accordance with state and federal regulations. To identify and provide for residents’ social, emotional, and psychological needs. To aid in the development of the resident’s full potential. To assist in the planning for the resident’s discharge. To provide service coordination. To provide family counseling, as needed. Performs all duties using independent judgment and discretion to implement regulations and policy.
Qualifications:
•Licensed Social Worker preferred.
•Preferred to have a bachelor’s degree in a human service field and one
year of supervised social work experience in a health care setting – preferably
with the psychosocial aspects of the elderly.
•Must possess a genuine caring for and interestin the elderly and physically
and mentally challenged people.
•Must be able to relate positively to residents, their families, employees, and
the community at large.
•Must be able to communicate effectively.
•Working knowledge of nursing home regulations, including MDS procedures, and
reimbursement and eligibility guidelines under Medicare and Medicaid.
•Must meet all federal, state and local health regulations and pass a
post-employment-offer physical exam. This requirement includes drug
screening, criminal background investigation, and reference inquiries.
•Must have the ability to read and write English.
•Must be capable of performing the Essential Job Functions of this job with or
without reasonable accommodation.
