Social Services Director - Nursing Home jobs in Rochester, NY

Newark Manor Nursing Home is seeking a highly motivated and compassionate Social Worker to join our team as our Director of Social Work . The ideal candidate will have knowledge of Medicaid and Medicare insurance and be able to work closely with residents and their families.

Only a short drive to a scenic location - work close to home in Wayne County, or only a short 30 min drive from Rochester, NY!

Job Description:

Coordinate with the admissions team to ensure that residents are admitted to Newark Manor in a timely and efficient manner

Ensure admission agreements are completely timely

Help residents and their families understand the benefits and limitations of Medicaid and Medicare insurance

Work closely with the nursing staff to identify and address the social and emotional needs of residents

Assist with discharge planning, including arranging for home health services or other necessary services

Conduct social assessments and develop care plans based on the needs and preferences of each resident

Provide emotional support and counseling to residents and their families

Help residents participate in recreational activities and social events

Coordinate with community organizations to provide additional support to residents

Requirements:

Bachelor's degree in social work or related field required.

Knowledge of Medicaid and Medicare insurance

Ability to work closely with residents and their families.

Excellent communication and interpersonal skills

Ability to work independently and as part of a team.

Familiarity with MDS

Pay: $20.00 - $24.00 per hour
