SOCIAL WORKER Job Summary

• Coordinate with the resident and resident representative to review policies on Advanced Directives.
• Completes MDS (Minimum Data Set) and UDA assessments.
• Schedule Quarterly and Annual Care Conferences.
• Arranges for appropriate services to meet the needs of the residents i.e. psych, dental, vision, etc.
• Begins discharge planning on day one. Assists residents with finding appropriate housing, arranging for appropriate equipment for discharge back into the community.
• Assists residents and families cope with the effects of the resident living a long-term care facility and temporary restricted visitation.
• Perform audits in response to reportable events and performance improvement plans.
• Ensure excellent communication among residents, families, and staff.
• Create positive resident and staff outcomes.
• Respect others by treating them the way we want to be treated.
• Demonstrate kindness, compassion, honesty, and integrity at all times.

REQUIREMENTS:
Degree in Social Work or Human Services

Successful criminal background check

Subject to pre-employment drug testing

Long-term care facility experience preferred

Proof of Covid-19 vaccination OR willingness to receive the Covid -19 Or Company approved medical /religious accommodation

Administrative/management experience preferred
