JOB SUMMARY
The primary responsibility of the Social Worker is to provide social work care management services to the participants of the Piedmont Health SeniorCare program; provides services such as participant screening, assessments, care management, supportive listening and coaching and referrals.
MINIMUM QUALIFICATIONS
Education/Experience: This position requires a master's degree from an accredited school of social work, 2 years professional level experience in a health related area and a minimum of 1 year experience working with the frail or elderly population.

Licensure, Registry or Certification Required: MSW.
Immunizations: Be medically cleared for communicable diseases and have all immunizations up-to-date prior to beginning employment.
