Social Work Manager plans and implements programs to meet the social and emotional needs of patients and patients' families in a health care setting. Develops plans for patient care after release from treatment. Being a Social Work Manager provides social services to patients/clients and their families. Collaborates with physicians, nurses, patients and their families to assess patient social needs and monitor progress of medical treatment. Additionally, Social Work Manager coordinates planning for post-discharge care. Supervises professional staff. Prepares departmental budgets and reports. Requires a master's degree of Social Work. Typically reports to a director. The Social Work Manager manages subordinate staff in the day-to-day performance of their jobs. True first level manager. Ensures that project/department milestones/goals are met and adhering to approved budgets. Has full authority for personnel actions. Extensive knowledge of department processes.     Job Role Summary

    Responsible for providing a continuum of social work services in an assigned work setting that will assist patients/clients and their families find practical ways of resolving social, emotional and environmental problems that interfere with optimum functioning and attainment of total health. Works closely with other community agencies and service providers to maximize health outcomes for individuals, families, and communities. The majority of services provided are home-based.

    Essential Duties

    • Provides therapeutic social work intervention to diverse client system which may include but not limited to crisis intervention, community resource referrals, client advocacy, support, outreach, follow-up, education, and intra/inter agency consultation.
    • Participates in a multidisciplinary team effort to coordinate/implement services that will ensure appropriate treatment plan continuity of care for all clients/patients served of care.
    • Serves as social work services resource/consultant person concerning appropriate management of social situations that affect the effective functioning of individuals and families.
    • Prepares written reports and clinical records (progress notes, summaries, statistical forms, etc.) of services rendered; responds timely to requests for information.
    • Assumes advocacy/liaison/collaborative role for patients/clients via contacts with community agencies/personnel, hospital personnel, court disposition hearings, abuse cases, etc.; collaborates with inter/intra agency personnel involving referral activities.
    • Maintains a current file of referral sources; cultivates a network of resources and collaborates with inter/intra agency personnel involving referral activities.
    • Patient/Client and their family’s records and information, staff records and/or their families, of all other confidential information presented orally and or in writing will be kept confidential.
    • Maintain confidentiality of patient/client records and information in accordance with HIPAA guidelines.
    • Assumes responsibility for maintaining current knowledge of information relevant to specialty area.
    • Functions as a skilled consultant concerning effective management of interpersonal situations that affect the positive coping skills of individuals, families, at-risk groups.
    • Documents services rendered via client records, statistical records, and written summaries.
    • Observes and Interprets impact on client functioning in environmental conditions and family interactions through home visits, office visits and community encounters.
    • Assists the Administrator of Social Work and Clinical Coordinator in program design and implementation of social work services, goals, and objectives.
    • Utilizes communication and problem-solving knowledge/skills to identify and facilitate resolution of problems in team functioning.
    • Participates in staff meetings, in-services, educational programs, and trainings; and assumes responsibility for continuous professional self-development.
    • Assists in developing and leading groups in the community, based on identified needs and evaluates the group’s effectiveness.
    • Participates on SW/ MCPHD & H&HC/Community committees/projects as required or assigned.
    • Frequent external contacts with patients/clients, families, health, and social service agencies concerning referrals, care coordination and acquisition/provision of confidential information.
    • Occasional requirement to testify in court cases (abuse/neglect, custody).
    • Perform other duties as assigned.


    Other Duties and Responsibilities:

    • Serve on appropriate committee and task forces as assigned.
    • Trained at the appropriate National Incident Management System (NIMS) and Incident Command System.
    • Training on Psychological First Aid (PSA) in preparation to participate in the response to public health disasters/emergencies.
    • Participate in Health Fairs.

    Qualifications

    • Bachelor’s Degree or Master's degree in Social Work, required.
    • A valid Indiana Driver’s License and access to a vehicle is required.

    Knowledge, Skills & Abilities

    • Graduation with bachelor’s degree in social work from an accredited School of Social Work.
    • 1-3 years of social work experience in a healthcare setting is preferred.
    • Knowledge of pertinent State/Federal laws, regulations, community systems, and resources.
    • Ability to work cooperatively with multi-discipline teams in situations requiring a high degree of flexibility, creativity, and initiative.
    • Apply social work techniques/principles including evaluative, intervention, counseling methods; toward productive problem solving in the client /worker relationship.
    • Communicate effectively with a wide variety of people, and perform duties independently in a wide variety of settings,
    • Knowledge and use of computer and skills for writing/documenting case notes, reports, e-mails, etc.

    Working Environment

    • Possible exposure to communicable diseases, infections; and other hazards, involved in home office, clinic, and community site visitations.
    • Effective response to stress situations; and appropriate response in dealing with difficult patients/clients/families.
    • In-county travel: Visits to office, community locations, and client’s school or home as needed or required.
    • Daily contacts with patients/clients, health, social work, and other professionals to initiate/provide/obtain services for patients/clients.
    • Frequent external contacts with patients/clients, families, health care and social service agencies concerning referrals, care coordination; and acquisition/provision of confidential information.
    • Occasionally required to testify in court cases (abuse/neglect, custody, environmental issues).
    • Patient/Client medical records cannot be transported home or to another work site without permission from the supervisor and in compliance with MCPHD HIPPAA guidelines and protocol.
    • All Patient/Client records and other client information must be stored in a secured locked cabinet in a safe and confidential environment.
    • Flexibility and high level of organizational skills to respond effectively to changing situations/priorities/locations.
