As a social worker with MACS Plan, you will play a crucial role in helping seniors and disabled adults to have more independence and greater quality of life. Your duties will include completing comprehensive medical, cognitive, and resource assessments to determine placement options for long-term care Medicaid clients.
You will also be responsible for placing Medicaid clients in a variety of settings, including home, with family, or assisted living placements. You will help develop a comprehensive care plan to address the medical, safety, and resource needs of clients at the identified placement sites. You will coordinate moves and services and attend case conferences with clients' families, facilities, and service and medical care providers on client's care and status. You will also participate in occasional and periodic team meetings, community events, and related training to stay up-to-date on the latest social work practices.
In addition, you will provide ongoing case management for clients to assure that needs are being met and quality care is being provided. You will travel to clients' residences to perform monthly/quarterly monitoring of clients' health and safety status, assuring that needs are being met and quality services are being provided. You will also document all services provided to clients and assess and reassess clients for program eligibility.
