Ohioans is looking for an innovative Medical Social Worker who wants to improve the independence and coping skills of home health patients while also assisting in the development of our expanding organization into hospice care.
The Master of Social Worker will implement the standards of care for medical social work services and collaborate with primary case managers to assess, plan, facilitate, and advocate options and services to most appropriately meet individual patient's needs. The medical social worker will assist primary case managers to improve patient's overall knowledge and compliance of their care plan.
