SOCIAL WORKER - FULL TIME Full Time Position. The Social Worker Assesses patients for appropriateness to Critical Assess setting. Ensures all criteria is met and patient is appropriately assigned to hospital and/or swing bed program. The social worker will make a comprehensive assessment of the patient’s needs, strengths, goals, life history and preferences and establishes a Plan of Care. Assists patient with needs within the hospital setting and refers patient to appropriate area for assistance outside of hospital. Works with physician and nurses to ensure patient is receiving all available services. Documents assessments, interviews, and Plan of Care. Ballinger Memorial Hospital District offers numerous outpatient services, acute and swing bed care, and is certified as a level IV trauma ER.
