Job Summary and Qualifications

The Medical Social Worker evaluates psychosocial needs and provides community service coordination for identified patient populations, providing support and crisis intervention where indicated. Acts as a liaison between the patient and community resources/post-acute care providers. The Social Worker creates and facilitates timely implementation of discharge plans and arranges post-acute interventions identified in the plan of care. The Social Worker facilitates patient movement across the continuum of care and identifies, addresses, and tracks barriers to patient throughput. In this role you will have the ability to positively impact and improve lives.

What you will do in this role:

• You will facilitate the interdisciplinary plan of care with a focus on evaluating the appropriateness of clinical care, medical necessity, admission status, level of care, and resource management.
• You will coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization.
• You will identify potential barriers to patient throughput and quality outcomes
• You will evaluate the psychosocial needs of patients and family support systems and coordinate appropriate discharge plans for identified patient populations.
• You will act as a liaison between the facility and resources external to the organization.
• You will help with the facilitation and implementation of timely discharge plans and facilitate follow-up to anticipated post-acute interventions identified in the plan of care
• You will provide crisis intervention and support to patient/family support systems.

What qualifications you will need:

• Master’s Degree in Social Work is required
• Licensure as a LMSW is preferred, LCSW is highly preferred
• 1-3 years of experience of inpatient hospital Medical Social Work is required
