The Social Worker is responsible for assisting patients and families with discharge planning, including, but not limited to, home health, DME, rehabilitation hospital, transfers and nursing home placements. Also serves as a resource for staff, patients and families with regard to community agencies. Manages adoptions and assists with special programs and education.
Bachelors Degree in Social Work required. Two (2) years experience in hospital or community social work. Case Management experience. Knowledge of medical and social work terminology. Uses a variety of standard office equipment.
Successful candidates will be subject to criminal background and drug screens.
