Licensed Master Social Worker / Bachelor Social Worker
JOB SUMMARY:
The purpose of the Social Work position is to deliver varied social work services to Endless Journey, LLC clients and their families. The individual will provide initial emotional, spiritual, psychosocial assessments, ongoing supportive services which include but are not limited to; counseling, patient and family education, outreach and referral. The Social Work position is an integral part of the Interdisciplinary Group process.

Supervised by: Executive Director

Interrelationships: Clients, family, IDG and hospice team members
Training/Licensure:
- Have an MSW degree from a school of social work accredited by the Council on Social Work Education (CSWE), and one year of experience in a health care setting.
- Have a bachelor’s degree in social work (BSW) from a school of social work accredited by the CSWE and be supervised by a MSW. If the BSW is employed by the Hospice before December 2, 2008, he/she is exempted from the MSW supervision requirement.
QUALIFICATIONS:
- Master's degree in social work from a Council on Social Work Education (CSWE) accredited university OR A BSW is required.
- Minimum of two years of experience in social work preferred.
- Knowledge of professional guidelines regarding confidentiality of client and staff information Preferred.
- Competent in the use of a variety of computer applications, including Microsoft Excel and Word required.
- Ability to work effectively with diverse patient groups and their families required.
- Must demonstrate excellent communication skills both verbally and in writing required.
- Must demonstrate personal traits of a high-level commitment, motivation and energy, team orientation, and professionalism required.
- Knowledge of health care services and resources required.
- Ability to work independently and in a team environment is required.
- Strong analytical skills required.
- Certified Social Worker (CSW) by Nebraska law (Or Eligible to Obtain) OR Provisional Certified Master of Social Work (PCMSW) required.
JOB FACTORS:
Physical Requirements:
Position requires minimal physical effort most of the day including kneeling, squatting, reaching, twisting, climbing, walking, exposure to temperature and humidity changes. Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position. Employee must provide evidence of annual TB test and other state-required tests or examinations. Employees must be fully vaccinated against Covid-19 or provide a valid medical exemption form upon employment.
Mental Requirements:
Must be able to work independently, make judgments based on assessments and data available and act accordingly. Must be flexible, innovative and possess good interpersonal skills. Must be able to cope with mental and emotional stress and demonstrate emotional stability.
Working Conditions:
Be able to tolerate occasional exposure to elements including, but not limited to, odors, blood, body fluids and excrement, adverse environmental conditions and hazardous materials.
Transportation:
Must have a current valid driver’s license, auto liability insurance and reliable transportation.
Essential Functions:
- Performs initial psychosocial, emotional, spiritual and bereavement assessments and assists in the development and implementation of goal-directed IDG care plans.
- Conducts ongoing reassessments of client/family needs and counseling as required.
- Provides short term crisis intervention and individual or family counseling when indicated.
- Provides services, under the direction of a physician (who approves plan of care).
- Participates as a member of the Psycho-Social Team.
- Observes, assesses and brings to IDG conferences information regarding psychosocial, emotional, spiritual, physical and financial conditions affecting the client and family.
- Assumes the active role of advocate for the client/family unit.
- Develops and maintains contact with appropriate community partners, agencies and services in order to promote inter agency cooperation and to facilitate related referrals.
- Documents comprehensive psychosocial, emotional and spiritual assessment clearly and concisely in a timely manner.
- All client/family visits, telephone contacts and referral actions are recorded in the client record per policy, in a timely manner.
- Provides ongoing education and counseling related to issues of death and dying to the client and family as needed.
- Attends staff meetings, IDG and other meetings as assigned and appropriate.
- Adheres to all policies and procedures.
- Assumes responsibility for own personal and professional development and maintenance of skills in social work.
- Shares On-Call responsibilities with other members of the Psycho-Social team to ensure after hours and weekend coverage is met.
- Covers and arranges for social work colleagues in their absence to meet customer needs.
- Other duties as assigned.
