POSITION SUMMARY:
The Social Worker provides social work services to patients, their families and/or significant others through counseling, emotional support, assisting with environmental needs, crisis intervention, and linkage/referral, and discharge planning.
MINIMUM QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
1. Bachelor's Degree in Social Work.
2. Social Worker with current licensure to practice in the state of West Virginia.
EXPERIENCE:
1. Minimum 2 years of Social Work experience.
PREFERRED QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
1.Certification in Case Management (CCM or ACM).
EXPERIENCE:
1.Minimum (2) years' experience working in healthcare or in community welfare setting.

St. Joseph's Hospital of Buckhannon is looking for a compassionate and creative clinical social worker to join our team. The clinical social worker is responsible for providing support services in the community, office and hospital setting. The SW will be working with patients on the Obstetrics unit helping to identify and link to necessary resources allowing a successful discharge from the hospital. They will also work with infants and their families identified with substance exposures during pregnancy in the outpatient setting as a case manager. The Social Worker will assess needs and provide links to resources within the community. The ideal candidate has a valid state license, exceptional communication and and experience working with clients from a variety of backgrounds and demographics.
