Duties Include:

• Responsible for assessments, care planning & helping residents adjust to their new surroundings.

• Familiar with a variety of the field's concepts, practices & procedures.

• Relies on extensive experience & judgment to plan and accomplish goals.

•A wide degree of creativity and latitude is expected •Occasional Community Outreach

Requirements Include:

Candidates must have:

• Two 2 years nursing home experience, Minimum.

• A current/valid Bachelors Degree.

• Long-Term Care exp. preferred

• Exceptional interpersonal & leadership skills

• Knowledge of applicable state and federal guidelines

• Point Click Care experience, preferred; and a plus!
