Licensed Hospice MSW Casual The Licensed Social Worker assesses the psychosocial functioning of patients and families, and intervenes as necessary to connect patients and families to necessary resources and supports in the community; provide supportive counseling or grief counseling; help the patient to expand and strengthen their network of social support; and/or ensure appropriate access to care and resources and an environment that is safe for discharge. Psychosocial assessments, crisis intervention and linkage with community and financial resources may be provided for inpatients, outpatients, their families and significant others as appropriate. The Licensed Social Worker serves as a liaison to community groups and promotes educational programs focused on healthy behaviors. He/she may lead education and support groups as assigned.

Ability to use wide range of social work techniques and intervention modalities. Knowledge of state and federal programs that provide medical care and financial support to individuals. Knowledge of community resources. Ability to work with diverse patient and staff populations.
Must have excellent written and oral communication skills. Proficient in communicating clearly and effectively with multiple constituents. Excellent interpersonal communication and negotiation skills in order to interact effectively with all levels of hospital personnel. Demonstrated organization and prioritization skills. Skill in establishing and maintaining effective working relationships with physicians, hospital staff, external agencies and third party payers. Must have the ability to operate in a complex and constantly changing, dynamic environment.
Competent in the use of essential business applications: MS Word, Excel, PowerPoint, Outlook (or other email application).
